To escape the first room of level 18, THE COURTYARD:

It's deceptively easy; Just attack the lion's head switch
on the east wall of the room.  Use your fist, chainsaw,
pistol, shotgun (either one) or chaingun to activate the
switch-panel, and it will rise up, freeing you.
